One of the reasons that these errors are displayed could be when the Notes DB connector Jar files (Notes.jar and NCSO.jar) are missing from the java\lib folder.

Why is the Originator name not getting added to the document while performing the Add Document operation?

One of the reasons for this to occur could be when the Notes DB is configured locally without a password. To solve this, either define a password for the local setup or use the remote setup.
